強生宣布中國首個且目前唯一治療BRCA突變轉(zhuǎn)移性去勢抵抗性前列腺癌的復(fù)方制劑澤倍珂?在華獲批[1]

倍珂®是每日一次的口服復(fù)方制劑,用于聯(lián)合潑尼松或潑尼松龍一線治療攜帶BRCA1/2突變的轉(zhuǎn)移性去勢抵抗性前列腺癌成人患者,臨床證實其相較標(biāo)準(zhǔn)治療可顯著延長影像學(xué)無進展生存期[2]

北京2024年10月21日 /美通社/ -- 強生公司今日宣布,旗下創(chuàng)新治療藥物澤倍珂®(尼拉帕利阿比特龍片)正式獲得國家藥品監(jiān)督管理局批準(zhǔn)。作為目前國內(nèi)首個且唯一獲批的雙效復(fù)方制劑[1],澤倍珂®聯(lián)合潑尼松或潑尼松龍用于治療攜帶胚系和/或體系BRCA基因突變的轉(zhuǎn)移性去勢抵抗性前列腺癌成人患者(mCRPC)[3]

澤倍珂®是BRCA1/2突變mCRPC成人患者的一線靶向治療方案[4]。作為一種高選擇性聚腺苷二磷酸核糖聚合酶(PARP)抑制劑[5],尼拉帕利和醋酸阿比特龍的組合聯(lián)合潑尼松或潑尼松龍,能夠靶向mCRPC患者的兩種致癌驅(qū)動因素——雄激素受體軸和BRCA1/2突變[3],[6],[7]。經(jīng)臨床驗證,澤倍珂®聯(lián)合潑尼松或潑尼松龍可顯著延長BRCA1/2突變mCRPC患者的影像學(xué)無進展生存期(rPFS)。此外,與安慰劑相比,尼拉帕利還顯示出總體生存 (OS)改善的趨勢,可顯著延長至癥狀進展時間(TSP)和至細胞毒性化療起始時間(TCC),同時并維持了患者的生活質(zhì)量[8]。

近年來,中國前列腺癌的發(fā)病率顯著上升。據(jù)國家癌癥中心最新發(fā)布的2022年度中國惡性腫瘤疾病負擔(dān)報告顯示,我國前列腺癌的發(fā)病率為每10萬人中18.61例,已成為男性泌尿生殖系統(tǒng)中最常見的腫瘤[9]。盡管隨著我國醫(yī)療水平的提高,前列腺癌的治療已經(jīng)取得一定進展,但是mCRPC仍然是一種致命的疾病[10],[11]。據(jù)統(tǒng)計,大約10-15%的mCRPC患者攜帶BRCA1/2基因突變[12],[13],而攜帶BRCA1/2基因突變的前列腺癌往往惡性程度更高,可能具有更強的侵襲性和更高的轉(zhuǎn)移性疾病比例,患者的生存結(jié)局更差[14],[15],[16],[17],[18]。因此,NCCN和EAU等國內(nèi)外權(quán)威指南均推薦對mCRPC患者進行基因檢測,以提供更加精準(zhǔn)的治療決策,改善患者臨床獲益[19],[20]。

強生創(chuàng)新制藥中國區(qū)總裁Cherry Huang女士表示:"澤倍珂®的獲批再次印證了前列腺癌精準(zhǔn)治療時代的到來,突顯了基因檢測在前列腺癌診療中的重要意義。長期以來,強生始終關(guān)注不同疾病階段前列腺癌患者的切實需求,持續(xù)引入創(chuàng)新療法及產(chǎn)品組合,從前列腺癌的晚期治療,進一步覆蓋到更早期階段。同時,我們也希望讓更多患者實現(xiàn)前列腺癌全程管理,在早期、規(guī)范化、足療程的診療中獲得更長生存。"

此次澤倍珂®的獲批是基于一項隨機、雙盲、安慰劑對照的多中心III期MAGNITUDE研究。結(jié)果顯示,在BRCA突變亞組中,尼拉帕利聯(lián)合醋酸阿比特龍加潑尼松或潑尼松龍(AAP)顯著降低影像學(xué)進展或死亡風(fēng)險達47%(rPFS, HR=0.53;95%CI 0.36,0.79;p=0.0014)[3]。在第二次期中分析時,中位隨訪時間為24.8個月,與安慰劑聯(lián)合AAP的10.9個月相比,尼拉帕利聯(lián)合AAP治療BRCA突變亞組的中位rPFS為19.5個月(HR,0.55[95%(CI),0.39-0.78])[21]。此外,尼拉帕利在至癥狀進展時間(TSP)上有統(tǒng)計學(xué)意義上的獲益,與對照組相比,癥狀進展風(fēng)險顯著降低了46%(未達到中位數(shù)與23.6個月相比;HR=0.54,95% CI:0.35-0.85;P=0.0071)[3]。值得注意的是,試驗還觀察到,接受尼拉帕利聯(lián)合APP治療的BRCA1/2 基因突變 mCRPC 患者與接受安慰劑聯(lián)合AAP治療的患者相比,尼拉帕利在至細胞毒性化療起始時間(TCC)上達到統(tǒng)計學(xué)意義和臨床意義的改善(中位時間未達到 vs. 27.3 個月;HR:0.56,95% CI:0.35-0.90; p=0.0152)[3]。

在安全性方面,研究顯示尼拉帕利和AAP聯(lián)合用藥與單藥的已知安全性一致。該聯(lián)合療法最常見的不良反應(yīng)(>10%)包括肌肉骨骼疼痛、疲乏、便秘、高血壓、惡心、水腫、呼吸困難等[3]
